Pacific Care Center is a 118 bed skilled nursing facility with an active transitional care unit for patients who are stable and no longer require hospitalization but who need an intensive program of rehabilitation and recover. The setting is also ideal for residents who need assistance with routine personal care activities and health monitoring by licensed nurses. Assistance and support is provided by a team of care givers whose goal is to become a positive part of the residents’ daily lives – helping them return home or assisting others in becoming as independent as possible. The facility’s design is open and bright - our inviting courtyard is a perfect retreat on a sunny, crisp afternoon! The room selection includes semi-private and 3 bedrooms. Meals and housekeeping services are included in the monthly service fee. Pacific Care Center is a certified provider of Medicaid and Medicare plans.
