BE AWARE of any statements from this doctor. Bogus claims have been sending out from this doctor's office. 3 people that I know that see this doctor have received statements saying they didn't pay part of their co-pay on separate visits from 3 years ago. Once you pay one statement, they immediately send another statement claiming co-pays were partially paid on other visits from years past.

Save all of your receipts of your co-pay or pay by credit card so you can prove that you paid!!!

law enforcement need to investigate this.
