TO GET STARTED YOU WILL NEED TO:
1--Write out a 3 day food record. Choose two days when you work and one day when you don't. It should indicate the time, what you TYPICALLY eat and how much you think you eat (example: 3 pm - 4 oz of grilled chicken with 1 cup of rice and beans) . It should include all beverages, even water.
2--Please fill out health history. Please contact our office for this form and fax or e-mail back.**Note: fax this (727-286-2977) prior to your initial appointment.
3--Please fill out the Metabolic Assessment Form (2 pages) and fax back. Please contact our office for this form.
4--Bring any blood work you may have or have your doctor fax it to Dr. Sheila Dean at 727-286-2977.
5--Bring all and any supplements, vitamins, minerals, herbs or anything of this nature with you (bring the actual container)
6--Bring a list of all medicines you may be taking and the dosages written out.
7--Bring the name, address and phone number of your primary care physician.

Training Dr. SHEILA DEAN, DSc,RD,LD/N,CCN,CDE is a registered and licensed dietitian, certified clinical nutritionist, certified diabetes educator and exercise physiologist. She received her undergraduate training through Rutgers University, completed her internship and graduate training with University of Rhode Island and Brown University's teaching hospitals and received doctoral training in nutritional genomics and pharmacology through the University of Medicine and Dentistry of New Jersey (UMDNJ). Sheila is a graduate of the Institute for Functional Medicine where she received advanced training in functional medicine and nutritional biochemistry. She has also worked with the Duke University Medical Center's Endocrinology and Metabolism Disorders Clinic and the Joslin Center for Diabetes as a certified diabetes educator.

Currently, Dr Dean is the owner of a medical nutrition private practice in Palm Harbor, Florida. An educator at heart, she served as an adjunct nutrition science professor at University of Tampa for 16 years. Dr. Dean has conducted hundreds of public presentations for groups from preschool age to the professional, has written continuing education credit modules for both nutritionists and nurses, has authored book reviews and articles for The St. Petersburg Times, Ms.Fitness magazine, Ironmanlive.com website and has appeared on NBC - News Channel 8, NBC-Daytime, CBS-News Channel 10, and ABC-News Channel 28 for dozens of interviews. She is also a contributing author of the medical textbook Integrative Gastroenterology published by Oxford University Press and the clinical nutrition textbook Krause's Food & Nutrition Therapy.She's served as the consulting sports nutritionist for the Philadelphia Phillies, has consulted for the Tampa Bay Buccaneers and was the media spokesperson and columnist for the Ironman Institute.

Sheila is the author of Nutrition & Endurance: Where Do I Begin?, (Meyer & Meyer Publishing) and is a certified health and fitness instructor with the American College of Sports Medicine (ACSM), the Aerobics and Fitness Association of America (AFAA), and the YMCA of America.

Dr. Dean is a good listener and was well prepared for our initial consultation. I could tell that she had thoroughly reviewed my pre-appointment info. She made sure to explain the options well and made sure every single question I had was answered. She responded to my emails promptly with clear answers. Dr. Dean was able to get to the bottom of my problem with few steps. She is very knowledgeable and writes down everything for you in a note so that you don't have to wonder what she recommended later.

It is obvious that their office is cleaned regularly. It was fantastic. I was amazed at how wonderfully they treated each of their staff members. They were so appreciative and thankful. I've never seen any employer treat their staff better, and it made me want to come back that much more. When comparing them to others I have visited for the same condition, they are among the best I have seen. They always make sure to return my phone call as soon as possible. They've never left me waiting. Their staff is the most friendly, welcoming staff I've ever dealt with. They are so warm and professional, and make the whole experience that much better.
